6. Do I need to file a Form 1583 for my spouse?

Yes, if your spouse will be receiving mail at the private mailbox or virtual mailbox, they will need to file their own Form 1583. As of the updated Form 1583 released by the United States Postal Service (USPS) in April 2023, it is now required that each individual, including spouses, who wishes to receive mail through a Commercial Mail Receiving Agency (CMRA) must submit a separate Form 1583. This requirement ensures that the CMRA has the proper authorization to receive and handle mail for each person.

Your spouse will need to complete their own Form 1583, providing two forms of identification to verify their identity. The form must be notarized if not signed in the presence of the CMRA/mail center operator. This verification process helps prevent mail fraud and ensures that only authorized individuals can access the mailbox.
